STAR Physical Therapy is committed to providing the highest quality rehabilitative care at the lowest cost. The Direct Referral Line is another example of that commitment. The Direct Referral Line is a simple way for physicians, case managers, and other referral sources to schedule patients for rehabilitation. To schedule patients, simply call or fax:

- Patient forms can be downloaded or printed on the "Patient Info" page. Complete the information or provide the same information from the referring physician, and fax the information to us at the number listed above.
- Initial evaluations for all patients will be scheduled within 24-48 hours.
- Early or late appointments will be scheduled if the patient is working modified schedules or full time.
- STAR will work with each patient to find the most convenient clinic for his or her needs.
- Appointments can be scheduled with the individual clinics, but the Direct Referral Line saves time, and there is no additional fee for utilizing this service.
- Once patients are scheduled, they will receive a fax or e-mail confirmation.
